For many organizations, internal audits have become little more than a checkbox exercise. The audit gets scheduled, a few records are reviewed, findings are documented, and everyone moves on.

Use this checklist to evaluate the effectiveness of your internal audit program. If you answer **"No"** to any item, it may be an opportunity for improvement.

**Audit Planning**

☐ Audits are scheduled based on risk, not just an annual calendar

☐ High-risk and problem-prone processes are audited more frequently

☐ Audit objectives and scope are clearly defined   

☐ Recent changes, customer complaints, and previous findings are considered during planning

**Auditor Competence**

☐ Internal auditors have received formal audit training

☐ Auditors understand the requirements of applicable standards

☐ Auditors are objective and independent of the areas being audited

☐ Auditors receive periodic refresher training

  

**Audit Execution**

☐ Auditors use process-based auditing techniques

☐ Employees are interviewed to verify process understanding

☐ Audit samples are sufficient to assess effectiveness

☐ Audits focus on both compliance and process performance

☐ Opportunities for improvement are identified and documented

**Findings and Corrective Actions**

☐ Findings clearly describe the requirement, evidence, and gap

☐ Significant findings receive root cause analysis

☐ Corrective actions address causes rather than symptoms

☐ Responsibilities and due dates are assigned

☐ Corrective actions are verified for effectiveness before closure

# ***Scoring Your Audit Program***

**21–24 Checks:** Excellent — Your audit program is likely driving continual improvement.

**16–20 Checks:** Good — Some opportunities exist to improve effectiveness and consistency.

**10–15 Checks:** Needs Attention — Your audit program may be identifying issues but not driving meaningful improvements.

**Below 10 Checks:** High Risk — Consider reviewing your audit approach, auditor training, and corrective action process.

**Bonus Tip:** If the same finding appears in multiple audits, don't ask, *"Why did this happen?"* Ask, *"Why didn't our system prevent this from happening?"* That shift in thinking often uncovers the true root cause.
